| Community Link Workers
The Community Link Worker project is a really exciting opportunity, not only to improve the lives of individual,
vulnerable older people, but also to demonstrate the value of low-level preventative services, particularly those
offered by the voluntary, community and faith sectors in Sheffield. A new team of 16 neighbourhood-based workers are
now in post and aim to support 6,000 older people over the next two years.
What does a Community Link Worker do?
They work in a local neighbourhood, in partnership with local organisations to:
1) Support individual older people to access the services, resources and facilities they need to maintain their
independence and quality of life.
2) Work with local groups and organisations to help them develop the services, resources and facilities they
offer to older people.
3) Build links between professionals, voluntary, community and faith organisations and individuals that will
last once the project has come to an end.

The 16 neighbourhoods were carefully chosen. Using information about, for example, how many older people live in
each area, how often they are admitted to hospital and how many have dementia, we believe the Community Link
neighbourhoods are the areas where the Community Link Workers will be able to make the most difference to the lives of
older people.
Who can they help?
Any older person, living in one of the Community Link neighbourhoods who is socially isolated or has physical or
mental health difficulties.

The Community Link Worker project is one of a number being run in Sheffield as part of Partnerships for Older
People projects, funded by the Department of Health. It is operated by Age Concern Sheffield under contract to
Sheffield City Council.
Community Profiles - more about the 16 neighbourhoods
Community Link Workers have produced descriptions of each of the neighbourhoods in which they work. Bringing
together demographic information and details of the services and resources available to local residents these Community
Profiles provide a picture of what it is like to live in each neighbourhood. These are working documents that will
continue to evolve throughout the project, however you can read the current versions below.
